I suspect to some, "airplane mode" and "off" feel about the same.
However, if someone is still using the phone (e.g. playing games,
listening to music), then a more sensitive person could react to
the electronics in the phone, even when in "airplane mode".

I suppose it depends on how you measure "safest". Some people
use SAR readings for that, although I am not convinced that SAR
readings are correlated to the reactions that a person with ES would
get.
People with ES tend to say that iPhones are the worse, Droids
are better, and 2G non-smart-phones are the best.
